Portuguese NIF Order Form

1. 护照或欧洲身份证复印件 Copy of Passport or European ID card

护照或身份证的扫描图像。您的签名必须是可见的。A scanned image of your passport or ID card. Your signature must be visible.

 

2. 地址证明 (PDF 版本) Proof of address (PDF Version)

这可以是银行对账单、水电费账单或驾照的第一页。该文件应包括您护照上的姓名和实际地址。

This can be the first page of a bank statement, a utility bill, or a driver's license. The document should include your name, as presented on your passport, and physical address.

 

- 必须是葡萄牙境外的居住地址。Must be a residential address outside of Portugal.

- 我们不接受 P.O.框或商业地址。We do not accept P.O. boxes or business addresses.

- 如果文件不是英文，请提供英文翻译 Provide an English translation if the document is not written in English

 

3. 为未满 18 岁的未成年人申请 NIF？Requesting a NIF for a minor under the age of 18?

- 未成年人的出生证明 Birth Certificate of minor

- 第一父母的护照复印件 Copy of first parent's passport

- 第二位父母的护照复印件 Copy of second parent's passport

 

4. 如果您出生在葡萄牙和/或持有葡萄牙国籍，您需要提供葡萄牙签发的出生证明复印件。如果您没有，我们可以为您索取一份。If you were born in Portugal and/or hold Portuguese citizenship, you will need to provide a copy of your Portugal-issued birth certificate. If you do not have one, we can request one for you.

请提供葡萄牙护照文件  Please provide the passport documents *

要求 Requirements:

- 4个边角清楚 Show all four corners

- 没有任何阴影和遮挡 No reflections or shadows

- 必须是在有效期内的（备注：不能过期） Must be valid (i.e not expired)

- 签字必须清晰可见 Signature is visible

2. 居住地址和地址信息* Residential Address and Proof of Address*

要求：Requirements:

- 必须是葡萄牙地址以外的地址 Must be a residential address outside of Portugal.

- 我们不接受P.O盒子或者商业住址 We do not accept P.O. boxes or business addresses.

- 请提供英文翻译件，如果文件不是英语文件 Provide an English translation if the document is not written in English

1. 驾照 Driver's license

2. 银行对账单首页 First page of bank statement

- 过去 3 个月内发行 Issued within the past 3 months

- 银行余额和交易详情可以会被涂黑 Bank balance and transaction details may be blacked out

 

3. 水电费 Utility bill

- 过去 3 个月内发行 Issued within the past 3 months

- 名字和姓氏必须与您护照上的姓名一致 - First name & surname must match the name on your passport

- 如果文件不是英文，请提供英文翻译 - Provide an English translation if the document is not written in English

If requesting a NIF on behalf of a minor, please include a proof of address of one of the minor's parents.

申请 NIF 的费用为 200 欧元（一个人），400欧元（一家两口），600欧元（一家三口）。包括一年（12 个月）的财政代表。在那年年底，您的财务代表将自动以每年 200欧元的费率续订。您可以在成为葡萄牙税务居民后取消此服务，并向税务局更新您的地址。The cost to request your NIF is €200 and includes one year (12 months) of fiscal representation. At the end of that year, your fiscal representation will automatically be renewed at a rate of €200/year. You may cancel this service after you become a Portugal tax resident and update your address with the tax office.
